This Telehealth Informed Consent explains what to expect when you use Privitals to connect with a licensed healthcare provider through our clinical partner, Qualiphy.
1. What Is Telehealth?
Telehealth allows you to be evaluated by a licensed healthcare provider remotely, without an in-person office visit. Your evaluation, including any medical questionnaire and provider review, takes place on Qualiphy’s secure platform.
2. Provider Evaluation
A licensed provider will review the information you submit and determine whether treatment is medically appropriate for you. Treatment is never guaranteed. The provider may approve, deny, or request additional information before making a decision.
3. Not for Emergencies
Telehealth through Privitals is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.
4. Benefits and Risks
Telehealth offers convenient access to care but has limitations compared to an in-person exam, such as the inability to perform a physical examination. As with any medical evaluation, there are inherent risks, including the possibility that a condition may be missed. Your provider will discuss any relevant risks specific to your treatment.
5. Confidentiality
Information you share during your evaluation is protected under HIPAA and Qualiphy’s privacy practices. Privitals does not have access to your medical records or evaluation details beyond what is necessary to fulfill an approved order.
6. Your Right to Withdraw
Participation in a telehealth visit is voluntary. You may decline treatment or stop the process at any time without affecting your ability to seek care elsewhere.

By proceeding with a visit through Privitals, you acknowledge that you have read and understood this consent, that you are requesting a telehealth evaluation voluntarily, and that you understand its risks and limitations.
